I, hereby give authorization

to , my Employer, to deduct

from any wages earned or to be earned by me as your employee, such sums equal to the dues and initiation fees in the amounts fixed in accordance with the By Laws of International Brotherhood of Electrical Workers (IBEW), Local Union No. 1 and the Constitution of the IBEW, and I hereby assign such amounts to the Union and direct you to remit and pay such amounts to the Union. I understand that such deductions will be made from my payroll checks each month. I also understand that the amount of dues and initiation fees may be adjusted from time to time in accordance with the By Laws and Constitution.

This authorization shall remain in effect for, and shall be irrevocable for, a period of one year from the date hereof, without regard to whether I am or remain a member of the Union during that period. If any term of this authorization is invalid or unenforceable, such term shall be excluded from this authorization to the extent of such invalidity or unenforceability; all other terms hereof shall remain in full force and effect. This card supersedes any prior authorization for dues deduction that I signed.

I recognize that my authorization of dues deduction is voluntary and is not conditioned on my present or future membership in the Union and I affirmatively consent to pay. Dues covered by this authorization are not deductible as charitable contributions for federal income tax purposes.

INFORMED AUTHORIZATION FOR USE OF DUES

FOR CONTRIBUTIONS AND EXPENDITURES

I, hereby give my informed

authorization to International Brotherhood of Electrical Workers (IBEW), Local Union No. 1 to obtain and use any portion of dues or fees paid by me to make contributions as defined in Section 130.011, R.S.Mo., or expenditures as defined in Section 130.011, R.S.Mo. This authorization shall remain in effect for, and shall be irrevocable for, a period of 12 months from the date hereof, without regard to whether I am or remain a member of the Union during that period. If any term of this authorization is invalid or unenforceable, such term shall be excluded from this authorization to the extent of such invalidity or unenforceability; all other terms hereof shall remain in full force and effect. This card supersedes any prior authorization for use of dues for expenditures and contributions that I signed.
